Clinical Laboratory Improvement Act (CLIA) Waived Testing Model Policy
    Residential care providers must comply with the waived testing requirements of CLIA.  This model policy was developed through the Medical Practices Coordinator to offer technical assistance to our providers to achieve compliance.  Contracted residential care providers are encouraged to customize this model for use in their facilities and corporations.

Infection Control Reporting:

Providers of personal care and community living supports services in licensed residential facilities are required to notify the Bay-Arenac Behavioral Health Infection Control Coordinator of all reportable infections as soon as possible, but not to exceed seven calendar days from the onset of the infection.  This requirement includes infections of recipients of care and staff. 

Infection reports may be phoned in by calling 989-895-2292 or by e-mailing the Infection Control Coordinator.  The report may also be made via fax to 989-895-2390 using the Infection Surveillance and Control Reporting Form.

If there are no reportable infections for a calendar month, providers must fax the Infection Surveillance and Control Reporting Form indicating that there were no reportable infections as of the end of the month.  These reports are due to the Bay-Arenac Behavioral Health Infection Control Coordinator no later than the fifth business day of each month.

Specialized Residential Quarterly Home Inspection and Maintenance Report
    This report is required for certain sites under contract with Bay-Arenac Behavioral Health and must be submitted to the Environment of Care office quarterly.
